Acknowledgment of Risk:

By participating in classes, activities, or open studio time at Milkweed Arts LLC, I understand and acknowledge that these activities involve inherent risks, which may include but are not limited to injury, property damage, or even death. These risks may arise from various sources, including the use of equipment, tools, materials, and interaction with other participants.

Release of Liability:

In consideration of being allowed to participate in activities at Milkweed Arts LLC, I voluntarily assume all risks associated with my participation. I agree to release, waive, discharge, and hold harmless Milkweed Arts LLC, its owners, employees, instructors, agents, and representatives from any and all liability, claims, demands, or causes of action related to any injury, loss, or damage sustained by me or my property during these activities, whether caused by the negligence of the releasees or otherwise.

Medical Treatment:

In the event of an accident or injury, I authorize Milkweed Arts LLC to seek emergency medical treatment on my behalf, and I agree to be responsible for the costs of such treatment.

Photography Release:

I grant permission to Milkweed Arts LLC to photograph or record video during activities for promotional purposes, including but not limited to use on websites, social media, and marketing materials. I understand that I will not receive compensation for the use of these images.

Indemnification:

I agree to indemnify and hold harmless Milkweed Arts LLC from any claims, actions, suits, procedures, costs, expenses, damages, and liabilities, including attorney’s fees, that result from my participation in any activities at Milkweed Arts LLC.

Acknowledgment of Understanding:

By signing in for open studio or any other activity at Milkweed Arts LLC, I acknowledge that I have read and understood this liability waiver and release form, and that I am agreeing to these terms voluntarily. I understand that this is a complete and unconditional release of all liability to the fullest extent permitted by law.
